We believe that each individual employed by us is entitled to be free of discrimination and harassment in the workplace. Sexual harassment is defined as: 1) unwelcome physical contact; 2) sexually explicit language or gestures; 3) uninvited or unwanted sexual advances; 4) an offensive overall environment, including the use of vulgar language, the presence of sexually explicit photographs or other materials, and the telling of sexual stories. Sexual harassment can come from superiors, fellow employees or customers. Men as well as women can be victims of sexual harassment. American Personnel & Temps will support any employee experiencing harassment and will intercede to rectify that situation. These and all personnel matters are handled in a confidential manner. If harassment occurs, please notify your staffing coordinator or manager of American Personnel & Temps immediately. A representative of American Personnel will file an Incident report and intercede in your behalf to correct the problem. Once the matter has been thoroughly investigated and if the allegation is sustained, the responsible employee will be disciplined. If the harassment continues, or if there is retaliation, the responsible individual will be terminated immediately!!
DRUG SCREEN POLICY
I understand that according to the American Personnel & Temps policy, I may be required to submit a sample of urine and/or blood for chemical urinalysis. I understand that this analysis will be collected and processed by a qualified laboratory for the purpose of determining presence of illegal drugs. I consent freely and voluntarily to the request for a urine and/or blood specimen. I hereby and herewith release American Personnel & Temps from any liability whatsoever arising from this request to furnish the urine and/or blood sample, and the decision made concerning my application for employment or continued employment based on the result of the analysis. I give my permission to American Personnel & Temps to release the results of these tests to any client company I am assigned to work with on a need-to-know basis.
AGREEMENT
In submitting this application for employment, I authorize investigation of my background, all statement contained herein, and it is understood and agreed that any misrepresentation (including omission of information) by me in this application will be sufficient cause for cancellation of the application and/or for separation from the Company's service if I have been employed. Upon written request, additional information as to the nature and scope of the investigation, if one is made, will be provided.
I understand that my employment with American Personnel & Temps is at will, and can be ended if deemed necessary by American Personnel & Temps. No person has the authority to modify the at will statement except in writing and it must be authorized by the President of the company.
I have received a time card and literature and understand the following about being an employee of American Personnel & Temps: 1) I have been informed and understand all policies and procedures; 2) I recognize that I am an American Personnel & Temps employee and agree to obtain permission from American Personnel & Temps for all absenteeism, call American Personnel & Temps if my supervisor discusses with me either permanent employment or a call back for an additional assignment; 3) I will give a one-week notice if I cannot complete my assignment; 4) Failure to provide American Personnel & Temps with a phone number is grounds for immediate termination; 5) I understand it is my responsibility to call the available line (409) 813-2008 within 24 hours after my assignment is completed and to call the available line each day with availability at American Personnel & Temps. Failure to do so will be considered a voluntary quit and can affect my eligibility for unemployment compensation; 6) I agree to never accept employment with any customer or agency within one year, where American Personnel & Temps has employed me, without prior written approval, I agree to pay 1% per thousand of my yearly estimated wage not to exceed 15% to American Personnel & Temps.
AGREEMENT ADDENDUM
I do hereby understand and agree accordingly to American Personnel & Temps policy, once I have accepted an assignment then neglect to be present at the date and time agreed, or fail to complete my assignment as agreed, without first notifying American Personnel & Temps each day with availability. I do understand failure to do so will be considered a voluntary quit and can affect my eligibility for unemployment compensation.
I do hereby understand it is my responsibility to call the available line (409) 813-2008 at American Personnel & Temps each day with availability. I do understand that failure to do so will be considered a voluntary quit and can affect my eligibility for unemployment compensation. |
